Annual Plan:

• Individual: $6.62/month
• ‍Two Person: $13.06/month
• ‍Family: $20.60/month

Basic Plan Benefits:

• Eye Exam: $10
• ‍Basic Lenses: $10 (single, bifocals, trifocals)
• ‍Frames: $130 allowances (any frames) plus 20% off any remaining balance
• ‍‍Contact Lenses: $130 allowance and 15% off any remaining balance
• Discounts on upgraded lenses also included.
